Cemented Bipolar Hemiarthroplasty for the Femoral Neck or Cervicotrochanteric Fracture in Patients Older than 85 Years

Abstract
Purpose : This study was performed to analyze the outcome of cemented bipolar hemiarthroplasty for the femoral neck or cervicotrochanteric fracture in elderly patients older than 85 years.

Materials and Methods : From January 1995 to May 1999, 22 patients over 85 years underwent cemented bipolar hemiarthroplasty for the femoral neck or cervicotrochanteric fracture. We evaluated postoperative Harris hip score (HHS) and daily activity of life as a clinical assessment and used Kelly¡¯s cement scoring system as a radiological assessment.

Results : Mean postoperative HHS was 73 points. In rating the daily activity of life among the 22 patients, 12 patients (54%) reported postoperative results as being above good. According to the Kelly (Ed- or ¡¯Kelley¡¯s¡¯)cement scoring system, 95% of patients were evaluated as being above good. The analyses showed that patients who had underlying disease compromising ambulation had worse result than those who were relatively healthy in HHS and daily activity of life, to a statistically significant degree.

Conclusion : We found that operative outcomes of cemented bipolar hemiarthroplasty for the femoral neck or cervicotrochanteric fracture in patients over85 years were poor because they had many underlying diseases affecting gait.
